ACADEMIC SUSPENSION APPEAL FORM

Completion of the following form is required to request an appeal. After this form is submitted, the academic standing committee has met with the student, and if the appeal is approved, students will be required to do some or all of the following:

• Enroll in FYEX 0990 and complete with an 80% or better
• Complete mandatory Success Coach and/or TRIO advisor meetings
• Complete mandatory meetings with other college officials (scholarships, financial aid, tutoring, etc.)

At any time a student on Academic Suspension can request a meeting when support and guidance is needed for academic success.

Deadlines

• Fall Semester - August 1
• Spring Semester -  January 1
• Summer Term - June 1

Academic Suspension Appeal Process

Students who have been placed on Academic Suspension have the right to appeal for extenuating circumstances. Students must complete and submit the Academic Suspension Appeal Form by the date noted in the Academic Suspension email.

A student may appeal a maximum of two times before being Academically Dismissed.

I give ZSC Success Coaches and/or TRIO advisors permission to register me for FYEX 0990. FYEX IS ONLY OFFERED IN PERSON ONE HOUR PER WEEK. I understand that by adding FYEX 0990 I am financially responsible for the course and the addition will impact my billing and/or financial aid status. I acknowledge that any refund percentage for dropped coursework is based on the Refund Time Frame stated on My ZSC on the Finances tab. 

I give ZSC employees on the Academic Suspension Appeals Committee permission to review my appeal form and my ZSC transcript.
